REPORT NUMBER: A0 50-265/75.-37 REPORT DATE:

October 17, 1975 OCCURRENCE DATE: October 7, 1975 FACILITY: Quad-Cities Nuclear Power Station Cordova, 111.

61242 IDENTIFICATION OF OCCURRENCE:

}

Unit Two feedwat.er flush line leak.

t CONDITIONS PRIOR TO OCCURRENCE:

Unit Two was in the cold shutdown condi tion.

DESCRIPTION OF OCCURRENCE:

II:00AM on October 7, 1975, during a core maintenance outage, a routine shutdown Atinspection revealed minor leakage from the "B" feedwater flush line in the area where Immediately after the discovery the flush line is welded to the "B" feedwater line.

of the leakage maintenance personnel scheduled the required repair work.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure - The apparent cause of this occurrence is designated as equipment The investigation of the pipe leakage revealed that the flush line was not failure.

This rdequately supported and thus experienced vibration during normal operation.

vibration resulted in fati'gue failure at the point where the flush line taps into the "B" feedwater line.

ANALYSIS OF OCCURRENCE:

isolation valves.

The failed line was outside the boundary of the primary containment leak was isolated from the reactor vessel by the check-valves in the feed-Thus, the The supply of coolant to the reactor was not jeopardized since the water line.

the time of the occurrence. There emergency core cooling systems were operable at was no radioactivity released to the environs; thus, there was no effect on the health and safety of the public as a result of this occurrence.

CORRECTIVE ACTION:

the old weld material and rewelding the pipe The pipe was repaired by grinding outIn addition, based on a recommendation by Station Nu Junction.a metal reinforcing sleeve will be welded to the flush line and. pipe supports wi.ll be provided to decrease the vibration of the flush line during normal operation.:!

FAILURE DATA:

the Quad-Cities Station There have been several failures in the feedwater system atSolutions to the vibration probl due to vibration problems.

Several of these solutions have by the Station Nuclear Engineering Department.

been implemented, and proposed modifications to the feedwater system are scheduled for completion at the earliest opportunity.
