i The purpose of this letter is to provide you with notification of an Abnormal Occurrence as requi red by Techni cal Speci fica tion 6.6.B. I.a.
At 2:00AM on flovember 9,1975, with Unit 2 operating at 180 liWE, traversing 1
incore probe (TIP) ball valve number 2 failed to close following a complete TIP set.
Upon discovery of the occurrence, the key to the squib valve was issued to the uni t operator for fi ring the squib i f requi red for isolation.
The operator tried to manually close the ball valve several times from the control room with-out success.
Work request 4366-75 was issued to repair the valve.
At 1:30PM on November 9, the ball valve was closed.
