Dear Mr. Keppler:
The purpose of this letter is to provide you with notification of an Abnormal Occurrence as required by Technical Specification 6.6.B.I.a within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> of the occurrence.
On flovember 7, 1975, the Reactor Core Isolation Cooling (RCIC) system turbine tripped due to high exhaust pressure, during a Unit Two post maintenance outage surveillance test.
This was in violation of Section 3.5.E.1 of Technical Specifications.
The high backpressure was caused by the failure of the hjdraulic fluid servo valve actuator which controls the RCIC turbine inlet steam governing valve.
Excessive steam entering the RCIC turbine created a high turbina back-pressure and the turbine tripped.
The High Pressure Coolant injection (HPCI) system was tested and found operabic.
The servo-valve actuator will b2 replaced.
This occurrence was discussed by telephone with !ir. Knap of your office on flovember 8, 1975 Further inforrution will be provided by Abnormal Occurrence Report A0 5_0-265/75-W.bich will be submi t ted wi thin 10 days of the date of this occurrence.
