OCCURRENCE DATE: October 16, 1975-FACILI TY: Quad-Cities Nuclear Power Station Cordova, Illinois 61242 IDENTI FICATION OF OCCURRENCE:
- One Bergen-Patterson and two Grinnell hydraulic snubbers were found to have empty oil reservoi rs.
CON'DITIONS PRIOR TO O'CC'URRENCE:
Unit Two was in the cold shutdown condition for a maintenance outage.
DESCRIPTION OF OCCURRENCE:
On October 16, 1975 during routine surveillance of hydraulic snubbers on
- Unit Two, Grinnell snubbers No. 4752 on the main steam drain line and No.
1224 on the clean-up pump suction line were found to have empty oil reservoirs.
Bergen-Patterson snubber No. 157 located inside the Unit Two drywell on the four inch recirculation bypass line was also found to have an empty accumu-lator. Work Request No. 4120-75 was issued to repair and refill all three snubbers.
DESIGNATION OF APPARENT CAUSE:
Equipment Failure The apparent cause of this occurrence was due to equipment failure.
The two Grinnell snubbers had oil leaks which allowed the reservoi rs to drain.
The Bergen-Patterson snubber was also found to be leaking.
ANALYSIS OF OCCURRENCE:
Hydraulic snubbers are designed to prevent unrestrained pipe motion under dynamic loads during an earthquake or severe transient, while allowing normal thermal motion during startup and shutdown.
Since Unit Two did not experience any seismic or other initiating dynamio-loads during the period of operation since the last snubber survei.llance, there was little chance of structural damage occurring.
In addition, all three restrained pipe lines had operable snubbers adjacent to the leaky snubbers to restrain each pipe line in the event of a dynamic load.
At no time were safe plant operation or the health and safety of the public affected by this occurrence.

A0 50-265/75-39 October 24, 1975 CORRECTIVE ACTION:
The two Grinnell snubbers will be removed and either repai red or replaced.
New snubbers are available at the site.
These new snubbers will be used to replace the leaky Grinnell snubbers where they can be fitted into place.

in the event that either of the two Grinnell snubbers cannot be replaced, they will be repaired by replacing the seals and tightening the fittings.
The four inch recirculation bypass line inside the drywell that is restrained
' by the Bergen-Patterson snubber is being removed (A0 50-265/75-38) so this snubber will be removed f rom service and will not need to be replaced.
FAILURE DATA:
Quad-Cities Station has had problems with leaking snubbers in the past.
None of these problecs have resulted in any hazard to the plant personnel or the public.
It is felt that periodic inspections and timely repairs should minimize future occurrences of this type.
