On August 12, 1975, ultile perforralng routinc surveillance testing of the ECCS 10,4 pressure permissive sensors, PS-2-263-52A uns discovered in the isolated ccndition. The other three sensors were not isolated. PS-2-263-52A was put into service and all four switches were satisfactorily functionally tested.

This occurrence was discussed by telephone with fir. Tarrbling of your office on August 12 1975 Further Information will be provided by Abnormal Occurrence Report A0 50-265/75-30 which wi11 be submitted within 10 days of the date of thIs occurrence.

Gn Augus t- 17, 1975 whlic bringing Unit Two back to power following a r.uinten-cnce outogcc, the 6" to. 4" reducer on the downstream side of. Icu flo4 feedwater regulating valve, 2-643, severed due to. vibration incurred.while treasferring f rom the low flow feedsater regulating valve to. the main feedwater regulating v21ve.

Tho unit was manually scrammed and the reactor 1cvel was maintain.ed with the RCIC system. Although contaminated. water was introduced into the oil separator none was released to the envi rons.
