ATTACHMENT RE: IAEA REPORTABLE OCCURRENCE 86-05 Summar of Event:

On the evening of Sunday, August 17, several Instrumentation and Control (IRC) technicians and their supervisors went on top of the reactor head to repair the control rod in position 3-5. This repair was necessary to bring the reactor critical and was performed at Hot Standby conditions. During this repair job, an IRC technician caught his foot in the string for one of the IAEA seals and caused the string to snap and break. The IRC crew retained the seal and string, and they will be given to the IAEA inspection team which will be at Tu'rkey Point on August 21 and 22, 1986.

Results:

1.Seal number N-68350 was broken and removed from the reactor head.

2. The four remaining seals are still intact and on the reactor head.
3. The seal and string were removed from the Radiation Control Area and will be turned over to the IAEA.

Conclusion:==

There was no breach of the IAEA monitoring program of the reactor core because four seals are still intact on the reactor head.
