SPECIAL REPORT MAIN STEAM LINE RADIATION MONITOR PURPOSE Technical Specification 3.5, Table 3.5-5, item 13.d requires that if the Main Steam (MS) line radiation monitor (RAD-6426) is inoperable for greater than 7
- days, FPL shall prepare and submit a special report to the Commission pursuant to. Technical Specification 6.9.3 within 30 days following the event, outlining the action taken, the cause of the inoperability, and the plans and schedule for restoring the system to operable status.
EVENT AND ACTION TAKEN On January 30, 1989, at
- 1040, RAD-6426 was taken out of service in order to re-place the detector assembly.
The detector was removed and the detector check check source was turned over to the Health Physics Department.
A new detector was obtained by Instrument and Control personnel,
- however, the check source was deter-mined by HP personnel to be leaking and could not be returned.
A replacement check source was obtained and installed on January 31, 1989. During the period January 31,
- 1989, through February 9,
- 1989, the detector was replaced twice due to problems with the detectors while performing calibration.
The calibration procedure employs both a Cesium-137 stick source and a Krypton-85 check source during the course of the calibration.
The I&C personnel performing the calibration were repeatedly unable to obtain a proper response from the detector while using the sources.
On February 9,
1989 the calibration was completed satisfactorily, and RAD 6426 was declared back in service at 0850, on February 10, 1989.
During the time RAD 6426 was out of service, Steam Generator sampling and isotopic analysis were performed daily.
Based on the results of these
- analyses, the health and safety of the public were not affected.
CAUSE OF THE INOPERABILITY RAD 6426 was originally removed from service to replace a damaged cable and detector.
The monitor was out of service greater than 7 days because of repeated problems obtaining a proper response from the Cesium-137 stick source and the Krypton-85 check source.
Contributing to the duration of the inoperability was the incomplete identification of the action statement in the Equipment Out Of Service log (only the requirement for sampling was listed).
SCHEDULE FOR RETURN OF E UIPMENT RAD 6426 was returned to service on February 10, 1989, at 0850, following successful completion of the calibration.
On-shift briefings for the senior reactor operators on this special report and on making complete entries in the Equipment Out Of Service Log will be completed by March 31, 1989.
