SPECIAL REPORT DECEMBER 21, 1988 PIRE PROTECTION SYSTEM IMPAIRMENTS REQUIRING NRC NOTIFICATION PURPOSE:

Technical Specification (TS) 3.14.5.b requires the submission of a report to the NRC if a non-functional fire barrier penetration is not returned to functional status within 7 days. TS 3.14 also states that certain other conditions require NRC notification. In accordance with the above requirements, this special report is being generated in order to notify the NRC.

EVENT The three impairments listed below have been open in excess of TS permitted time limits.

Fire Protection Impairment (FPI) Number 2125 was issued on November 14, 1988, for the breach of penetration 054F-H001 by the removal of its plug in order to provide access for welding leads and hoses to repair a Component Cooling Water hanger.

This breach was closed on December 1, 1988, following repair of the hanger.

FPI 1766 was opened on July 1, 1988, during the performance of Fire Barrier surveillance O-SMM-016.3 due to voids in the grout of a penetration in barrier 045F.

This impairment was not included in any previous Special Reports due to an error in the notification process for overdue impairments. FPIs issued in 1988 were reviewed with no other failure to report an FPI being identified . A letter of instruction which will properly control this notification process will be issued by December 30, 1988.

FPI 2137 was opened November 21, 1988, due to damage incurred while chipping grout on the opposite side of barrier 011S.

CAUSE AND CORRECTIVE ACTIONS:

The above impairments were incurred during the performance of surveillances, plant modifications, or equipment maintenance. The plant continues to implement the compensatory measures as applicable .

The attachment identifies impairments which are now closed but were previously reported as open.
