SPECIAL REPORT FIRE PROTECTION SYSTEM IMPAIRMENTS REQUIRING NRC NOT1FICATION PURPOSE.

Technical Specification (TS) 3.14.5.b requires the submission of a report to the NRC if a non-functional fire barrier penetration is not returned to functional status within 7 days. TS 3.14 also states that certain other conditions require NRC notification. In accordance with the above requirements, this special report is being submitted in order to notify the NRC.

EVENT:

Fire Protection Impairment (FPI) 2477, opened May 5, 1989, is for Detector Alarm Point 28. FPIs 2479 and 2480, opened May 5, 1989, and FPI 2491, opened June 7, 1989, are for Thermolag breeches.

FPI 2500, opened June 8, 1989, is for Penetration 019W-E014.

CAUSE AND CORRECTIVE ACTIONS:

FPI 2477 is on an alarm point taken out of service for welding in fire zone 054; work is ongoing. FPIs 2479, 2480 and 2491, closed June 14, 1989, were for Thermolag removal to find the source of water intrusion into an electrical junction box. FPI 2500 is for an incorrect penetration seal and work is ongoing. The plant continues to implement the compensatory measures as applicable.
