V. PROBABLE CONSEQUENCES JF tie OCCURRENCE:

At the time of the occbrrence. LaSalle Statica Unit l'was in the startup rnode (mode 2) undergoing initial heatyp phase testing'. Temperature limi ts ,

is speci fied by T.S. 3. 7. 7 Table 1, were not exceeded in the Uni' 2 Division 2 Swi tchgear rcom during the time period the Vx system was i noperable. No equipment was degraded. Safe ope ~ ration of the facility was maintained'at all times. -

VI. CAUSE: ,

As.Ja result of $be%Zack Company 's notificaticn of a 10CFR21 concerning discrepancies

, re la t i n g t o we l de r q'ua l i f-! ca t i on s an d s hop travelers , CECO Q. A. initiated a y review of Zack' Quality related records, and the welds involved. As a result of the inspection, three sections of the Vx duct work were found to have visually unacceptable Ocids due to porosity and weld profile.
