LASALLE INSPECTION REPORT 50-373/99015(DRP); 50-374/99015(DRP)

Dear' Mr. Kingsley:

This refers to the inspection conducted July 29 through September 16,1999, at the LaSalle

Nuclear Power Station. The purpose of the inspection was to determine whether activities

'

authorized by the license were conducted safely and in accordance with NRC requirements. At

- the conclusion of the inspection, the findings were discussed with those members of your staff

identified in the enclosed report.

.Overall, y' ur staff demonstrated a good safety focus, and plant operations, for the most part,

o

were conducted appropriately. Human performance errors, however, were involved in two

'

automatic reactor shutdowns that occurred within a 13-day period. In s first automatic reactor

.

shutdown, main control room operator responte to reactor water level control problems was not

>

in accordance with abnormal operating procedures and training. In the second automatic

reactor shutdown, operations outside of established procedures, incomplete. communications,

inadequate pre-job briefs, lack of direct oversight and supervision of activities outside of the

~

main control room, and itadequate main control room annunciator response led to a feed water

. transient and subse9 nt automatic reactor shutdown during on-line turbine-driven reactor

- feedwater pump maintenance' activities.

Also,- challenges to main control room operatois continued to occur during this inspection period

due to material condition and troubleshooting issues. For example, the feedwater transient that

led to the Unit 2 reactor automatic reactor shutdown was initiated by a loose mounting plate on

a turbina-driven reactor feed pump control oil servo assembly.- During the subsequent Unit 2

T startup operators were further challenged when the rod sequence control system led to

unanticipated rod blocks and a 5-hour delay in the pulling of control rods, in addition,

maintenance personnel challenged control room operators when they took actions whbi were

not prescribed by an approved troubleshooting plan and caused a combined intermediate valve -

between a moisture separator reheater and a low pressure turbine to change position

unexpectedly. '

Based on the results of this inspection, the NRC has determined that one' violation of NRC

requirements occurred. This violation is being treated as a Non-Cited Violation (NCV),

consistent with Appendix C of tiie Enforcement Policy. The NCV is described in the subject

inspection report. If you contest the violation or severity level of the NCV, you should provide a

response within 30 days of the date of this inspection' report, with the basis for your denial, to

the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington

DC 20555-001, with copies to the Regional Administrator, Region Ill, and the Director, Office of

' Enforcement, United States Nuclear Regulatory Commission, Washington DC 20555-001.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its

enclosure, and your response, if you choose to provide one, will be placed in the NRC Public
