LASALLE INSPECTION REPORT 50-373/99012(DRP); 50-374/99012(DRP)
Dear Mr. Kingsley:
This refers to the inspection conducted June 23 through July 28,1999, at the LaSalle Nuclear
Power Station. The purpose of the inspection was to determine whether activities authorized by
'
the license were conducted safely and in accordance with NRC requirements. At the
,
conclusion of the inspection, the findings were discussed with those members of your staff
!
identified in the enclosed report.

Overall, your staff demonstrated a good safety focus and dual plant operations were conducted
appropriately. Operators conducted activities in the main control room conservatively and
safely. In particular, activities performed as a result of the lake temperature excursions were
timely and supported safe plant operations. The conduct of most engineering and radiological
control activities were performed correctly.
However, challenges to the main control room operators continued during this inspection period
due to degraded equipment issues. For example, the continued cycling of the Unit 2 D vacuum
breaker throughout the inspection period in conjunction with the failure of position indication on
the Unit 2 A vacuum breaker early in the inspection period required operators to take significant
compensatory actions, especially during Unit 2 emergency diesel generator testing. In addition,
some repeat items indicated corrective actions implemented to address deficient conditions did
not preclude recurrence. These included several partial actuations of the reactor protection
system including one where corrective actions from two prior instances within the last 6 months
were ineffective.
