Technical Specification 3.3 7 5 and 3.4.2. requires, the Safety / Relief Valve position indication to be operable. On ' July l21,' 1982 while performing LOS-MS-R2, Main Steam Safety /Relle f ;Vd,1ve Manu' l - Cycling a

Test, the position indication lights for ADS Val ~ve 1821-F0130 failed to operate properly. The position indication lights showed' the valve to be full open af ter the valve had been opened then closed.

The valve-was verified, locally at the valve, to be fully closed.

This was the first time that these valves had-been functionally tested.

PROBABLE CONSEQUENCES OF THE OCCURRENCE e,-

At the time of this event, l.aSalle Unit 1 was i,n Condition 2 undergoing,

initial heatup phase testing.

Observers were stationed 'in 'the drywell to monitor safety / relief valve operation.

in addition to the observers, the NSSS Alarm typer printouts for the S/RV position were functioning prope'rly and indicated-the valve was closed.

Safe op'eration of the t-Unit was maintained at all times.

VI.

CAUSE

The cause of this occurrence. was improper interaction between 'the magnet and the wired set of reed switches in the position Indication assembly for this valve.

The position incication assembly was manufactured by Crosby Valve and Gage Company.

<s Vll. CORRECTIVE ACTION 1

A work request, L17469, was ini tiated to correct _ the problem of the incorrect position' indication.

Under th}s work request, the posi tion indication was repaired by changing all electrical' connections to the spare set of reed swi tches. This SRV was' cycled, satisfactorily, and declared operable at 0300 on July 23, 1982.
