BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - RESPONSE TO VIOLATION 50-438, 50-439/82-14 MODIFICATION OF CLASS IE EQUIPMENT IMPLEMENTED BY VERBAL INSTRUCTIONS This is in response to F. J. Long's letter dated June 16, 1982, report numbers 50-438/82-14, 50-439/82-14, concerning activities at the Bellefonte Nuclear Plant which appeared to have been in violation of NRC regulations.

10 CFR 50, Appendix B, Criterion V, as implemented by Bellefonte's SAR Section 17.1A.5 requires activities affecting quality to be prescribed by documented instructions in the f orm of drawings, specifications, and procedures.

Contrary to the above, modifications on the safety-related cabinets in the auxiliary building for licensee identified items 43 8/ 81-41 and

  • 439/81-41 were accomplished by means of verbal instructions to craft personnel.

Admission or Denial of the A11eaed Violation TVA admits the violation occurred as stated.

Reason for the Violation Noncomformance report (NCR) 1472 was written against one balance of plant (BOP) isolator cabinet 2IX-ISOL-262-G. The disposition of the NCR specified that BLN CONST rework all BOP isolator cabinets supplied by E-Max, Incorporated. Contrary to the information supplied in the inspector's report, which stated that only one work release was issued, three work releases (#21653, 21727, and 27973) were issued, including two for isolator cabinets in the Auxiliary Building. The violation occurred as the result of responsible engineering personnel inadvertently not including all isolation cabinett on the issued work releases and directing the craf ts to perform the work without the proper authorization.

Corrective Action Taken and_Results Achieved All work performed on isolator cabinets was performed correctly, including that work performed without written instructions, in accordance with the approved disposition of NCR 1472. QCIR 20,458 was initiated on May 17, 1982, to document the unauthorized modification and work releases (#33370 and 34609) were initiated to document the rework. All work and QC acceptance of the . i solator cabine t s has ,been completed, and all documentation has been closed.

Steps Taken to Avoid Further Violations All responsible engineering and QC personnel associated with this work will be reindoctrinated in the necessity of documenting rework on any safety- l related equipment and not to direct craft to perform this work without proper authorization. This reindoctrination will be performed by July 30, 1982, and documented in accordance with BNP-QCP-10.29, ' Quality Assurance Training Program.' i l
