FINAL RESPQiSE TO SEVERITY LEVEL V VIOLATIQi 50-438/84-21-01, 50-439/84-21-01 FAILURE TO FOLLOW DRAWING FOR HV AC SUPPORT, EIECrIOi, AND INSPECTIOi Demription of Deficiency 10 GR 50, Appendix B, Criteria V and X, and the accepted quality assurance program (TV A-TR-75-1A), Sections 17.1 A.5 and 17.1 A.10 require in part that activities affecting quality be accomplished in accordance with drawings and that a program for inspection of activities affecting quality be executed to verify confermance with documented drawings. Bellefonte Nuclear Plant (BLN) drawing No. 4 AWC864-2-1, R4, Note 9 requires that heating, ventilating, and air-conditionir3 (HVAC) supports shall be fabricatM and erected in accordance with the American Institute of Steel Construction (AISC) specificaton,1969 edition as revised through 1974.

Contrary to the above, between August 1 and September 20, 1984, activities affecting quality were not being accomplished in accordance with documented drawings in that HVAC support No. Of C-298 did not meet the 1:500 levelness required by the AISC specification and a program for inspection of activities affecting quality was not executed to verify confonnance with documented drawings in that all HVAC supports were rot being inspected for this re qui rement .

Correc tive S tep s Taken Nonconfonnance Report (NCR) 3515 was initiatM by BLN Office of Construction (OC) to document the fact that the AISC specification did not adequately address the levelness requirements for HV AC duct supports. Specific pluub and levelness requirements have been incorporatM on a TVA T general note drawing. BLN site procedure (quality control procedure (QCP) 6.7) has been revised to mflect this criteria.

13f AC support No. OVC-298 has been reworked to meet the 1:500 levelness requimd by AISC specification. Additiornlly, BLN OC has performed a sample inspection involving 200 previously installed duct supports in accordance with MIL STD 105D and no discrepancies were fomd. TV A is in full compliance, and NCR 3515 has been closM.
