Results
Of the six areas inspected, no violations or deviations were identified in five

areas; one violation was found in one area ( Activ, ties affecting quality shall be prescribed by documented instructions, paragraph 5).

Licensee Action on Previous Inspection Findings a.
(Closed) Violation 50-438/81-09-01, Failure to Follow Procedure for Protecting Safety-Related Cables Adjacent to On going Construction Activities.
Tennessee Valley Authority letter of response dated April 10, 1981, has been reviewed and determined acceptable to Region II.
The inspector held discussions with licensee representa-tives and examined the corrective actions as stated in the letter of response.
The inspector concluded that TVA had determined the full extent of the subject noncompliance, performed the necessary survey and follow-up

actions to correct the present conditions, and developed the necessary-j corrective actions to preclude recurrence of similar circumstances.
The corrective actions identified in the letter of response have been l
implemented.

These corrective actions consisted of:
(1) The two specific examples were immediately corrected upon noti-ficatior, by the NRC inspector on March 4 and 5,1981.

(2) Craft and engineering supervisors were instructed to assist in
eliminating the type of violations. Documented evidence of this
action was noted during this inspection of May 11-14, 1982.

In addition, a tour of the cable spreading rooms and auxiliary building showed no violations of the same kind, b.
(Closed) Violation, 50-439/82-22-01, Cable Routing or Tagging Does Not Agree with Cable Pull Card. TVA letter of response dated October 29, 1981, has been reviewed and determined to be acceptable by Region II.
The inspector held discussions with the EEU QC Supervisor and examined the corrective actions ' as stated in the letter of response.
The inspector concluded that TVA had determined the full ext;ent of the subject noncompliance, performed the necessary survey and follow-up actions to correct the present conditions, and developed the necessary corrective actions to preclude recurrence of similar circumstances.
The corrective actions identified in the letter of response have been l
implemented.
These corrective actions consisted of:
(1) Quality Control Investigation Report 12,681 was written on September 9, 1981, to document the subject deficiency.
Work Release No. 23,703 was then prepared to lif t cables' 2 and 8 in order to investigate / correct the deficiency.
The cables were found to be routed correctly but tagged incorrectly.
The tags were then corrected and the associated work release was closed on September 11, 1981. Inspection of three similar cable installa-tions showed those cables to be tagged correctly.
(2) Termination inspectors have been cautioned to follow the require-ments of BNP-QCP 3.4 for verification of cable routing and correct identification (tagging).
In addition, the -individual inspector involved in this specific deficiency has been reinstructed in the requirements of BNP-QCP 3.4.

(Closed) Unresolved Item 50-438, 439/80-18-03, Seismic Qualification of Cable Tray Supports.
Quality Control Procedure 3.7,
" Electrical Hangers" initially had an acceptance criteria, based on Design Project
Manager's approval on Memorandum No. 761214086 dated December 14, 1976, that in essence allowed the modification of hangers without requiring a
,
reanalysis of the as-built support with regard to seismic qualifica-tion. As a ' result of the inspector's concern in this area, the subject procedure was changed to give more specific acceptance criteria and the I
licensee identified to Region II a 50.55e item concerning tolerance for
'
tubular steel cable tray hangers. This unresolved item is closed based on change to QCP3.7 and tracking of the 50.55e item is. covered by Numbers 50-438, 439/81-25-16 (CDR 81-4/ and 81-50).

was submitted on March 23, 1982.
The report has been reviewed and determined to 'be acceptable.
The inspector held discussions with responsible licensee representatives and reviewed supporting documen-tation to verify that the corrective action identified in the report has been completed or is in the process of being completed.
Field change Package 122, " Replacement of the C3 Capacitor in the Lambda Type LXS-E-24-R Power Supplies" has been completed for Unit 1 and Unit 2 is in process.
Inspector forms numbers85-100 per QCP 3.31 have been issued for replacing capacitors in Units 1 and 2.
Final QC inspection is in process, b.
(Closed) Item 438, 439/81-21-06 (CDR 438-81/43 and 439/81-45) " Defects in Foxboro Removable Manual Controller Modules - NCR's 4 & 5 (10 CFR 50.55(e))".
The final report was submitted on July 24, 1981.
The report has been reviewed and determined to be acceptable.
The inspector held discussions with responsible licensee representatives and reviewed supporting documentation to verify that the corrective actions identified in the report have been completed.

(0 pen) Item 438, 439/81-25-12 (CDR 438-81/41 and 439-81/43), "80P Isolator Cabinets".
This item is tracked at the site by NCR 1472, which identifies one cabinet by the plant's unique identification number system. This NCR implies that the problem may be generic to similar wall-mounted 80P cabinets.
Twenty-four cabinets requiring modifications were identified to the inspector by the licensee site personnel.
Approximately half were examined in the field to insure corrective action was complete and it was noted that final OC inspec-tion by the site was not complete.
The inspector requested the work release, work authorization, engi-neering change notice or other written documentation that directed the craft to make the modification of the subject cabinet.
It was i
determined that only one work release (#27973), which covered the
'
modification of cabinets in the in-take pumping station, was issued and j
that modifications of cabinets in the auxiliary building were i
accomplished by verbal instructions.

The above is in violation to 10 CFR 50, Appendix B, Criterion V which states activities affecting quality shall be prescribed by documented

IE Bulletin (IEB) (Units 1 and 2)
a.
(Closed) IEB No. 80-20, Failure of Westinghouse Type W-2 Spring Return to Neutral Control Switches. TVA letter of December 12, 1980, reports that the subject type switches are used in safety related circuits at the station. All of the switches are located on low-voltage switchgear boards, and the neutral position contact is not being used on any of the switches. However, a note will be added to the applicable low voltage-switchgear drawings stating that the neutral position contact shall not be used because of its potential unreliability.
TVA's.
Revision 2 to Westinghouse plan 1245F02 added a note to alert designers of possible problems if further needs require the use of this contact.
7.
Independent Inspection Effort The inspector conducted a walk-through inspection of the auxiliary building.
The current status in the electrical areas of the construction was observed.
Tne in place storage of several electrical components and motors was exam-ined. Equipment was adequately covered, heat was observed to be applied to motor winding, and oil levels of applicable equipment were satisfactory.
Within the areas examined there were no violations or deviations identified.
8.
Electrical (Cables and Termination III) - Observance of Work and Work Activities (Unit 1)
The inspectcr selected safety-related power cables 1EI-ECAS-1-A and IEI-ECAS-11-B for examination. The cables were examined to assure that the SAR and QAM requirements were complied with in the areas of receiving inspec-tion, storage, handling, identification, work and QC procedures, size and type of cable, location, routing, protection, separation, raceway supports and anchorages, raceway identification, cable pulling procedure, raceway loading, termination, and non-destructive testing.
Within the areas examined there were no violations or deviations identified.
Electrical (Cables and Terminations II) - Review of Quality Records (Unit 1)
The cables identified in paragraph 8 were selected for a quality record review to assure that the requirements of the SAR and QAM were complied with in the areas of receiving inspection, test reports, material certification, size and type of cable, location, routing, protection separation, raceway identification, cable pulling, terminations, raceway supports, and high-potential testing.
Within the area examined there were no violations or deviations identified.
