BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - SUPPLEMENTAL RESPONSE TO VIOLATION 438/84-03-01, 50-439/84-03 FAILURE TO FOLLOW PROCEDURES FOR CONCRETE EXPANSION ANCHOR BOLTS AND PIPE SUPPORT INSPECTIONS TVA's initial response to R. C. Lewis' letter dated February 29,-1984, report numbers 50-438/84-03, 50-439/84-03 concerning activities at the Bellefonte Nuclear Plant which appeared to have been in violation of NRC regulations was submitted on March 30, 1984. In that response, TVA indicated that the results of a survey to determine the extent of the missing washers on wedge bolts would be submitted upon its completion.
Below is this supplemental information.
A random sampling was taken of all sizes of wedge bolts installed from February 1980 to the present. A total of 86 pipe supports were selected for investigation of missing washers. These supports contained a total of 289 bolts with 9 bolts failing to have washers. Nonconformance Condition Report (NCR) 2833 was prepared to identify missing washers on wedge bolts.
The subject NCR was returned by TVA Engineering Design (EN DES) with a recommended corrective action to use-as-is. NCR 2833 has been closed.
Bellefonte Quality Control procedure (BNP-QCP) 2.8, " Bolt Anchors Set in Hardened Concrete," has been revised to ir.clude the requirement for wedge bolt washers as acceptance criteria.
