. w, HEQUIREMENTS FOR LOGS (V Y 1.0 GENERAL REQUIREMENTS l 1.1 The following general requirements apply to the maintenance of all Narrative Logs, Operating Logs, Rounds Sheets, and other records or logs kept by the Operations Department.

. When recording logs, complete and total accuracy is required. When signing logs, Operators are signing for the accuracy of all information contained therein, including individual actions taken 4

. On handwritten logs, errors shall be crossed out with a single line and initialed i _and dated by the person making the correction.

. Handwritten log entries shall be neatly written or printed in ink.

. Parameters exceeding specified maximum or below specified minimum values shall be circled in red and reported to the Control Room.

. When recording data from recorders

. For diaital displays, use the digits, not the trace.

. For mechanical displays, use the trace, not the pointer.

2.0 Shift Narrative Log -

The Shift Narrative Log is a written, or electronic, sequence of Unit status and events, used to maintain an accurate history of plant operations and to reconstruct important operating events.

NOTE The accuracy of times recorded in the Shift Narrative Log depends upon the amount of activity going on at the time and the importance of the event as well as the availability of other records to provide more precise information regarding the time of  ;

the event. For example, after a reactor scram when conditions have stabilized, the i Shift Narrative Log should show the exact minute of the scram event because the time can be obtained from the process computer. In contrast, the approximate time of placing a Condensate Demineralizer in service can be logged. However, even though the precise times may not be necessary in a log entry, it is important that the Q sequence of events be clear. Therefore, care should be taken that the approximate V times represent the approximate time intervals between events and that the events '

are logged in sequence as they occur.

. 2,1 Shift Narrative Log Format:

. The Shift Narrative Log shall be maintained using the computerized Shift Narrative Log Program. ,

. _ The top of each page shall be stamped to provide space'for shift supervision to initial indicating that pertinent information logged on that page has been read. l 1

. Shift Narrative Log heading shall contain the following information:

. Date

. Shift houc

. Names of personnel on shift

. Status of the Unit, including reactor power in %, Mwe, and ACTION  !

.- All Shift Narrative Log entries shall indicate the time the particular entry was accomplished.-

l 2.2 Shift Narrative Log Entries:  !

. Entries into the Shift Narrative Log may be made by the on-duty Unit Supentisor, -

Reactor Plant Operator,905 Operator, or Balance of Plant Operator, but shall i be reviewed by the on-duty SM before the SM signs the Log. l 1

. Log entries shall be detailed, describing events and plant status in a concise chronological manner. Enough information should be entered to ensure any scenario could be reconstructed at a later date.

. . Whenever possible, entries are made directly into the Shift Narrative Log. i i

. The following events and activities are representative of Shift Narrative Log entries:

A. Major equipment status changes:

1) The test results are either cut of specification, unsatisfactory, or not in accordance with expected results. Corrective actions (e.g.

TR #, contacted the system manager, etc.) should be logged.

2) The test was performed as the result of a TS Required Action, TRM Required Compensatory Measure, or REDODCM Required Compensatory Measure.

D. Any Reportable Event Reports initiated.

E. Log entry into and exit from any ONP or EOP procedure. Include sufficient information for the reader to determine why entry into these procedures was made.

V. All electrical switching operations.

W. Release or transfer of radwaste sample tanks.

X. Fire and medical emergencies, including drills.

Y. Log valid Control Room annunciator panel alarms which occur during the shift. Alarms which are known to be spurious, or are expected to occur as the result of a planned evolution, or are l expected to occur due to known plant component condition need not be logged.

)q 2.3 . Computerized Shift Narrative Log Program

. All Shift Narrative Log entries are automatically labeled based on user ID and password used during the logon process. Operators shall maintain Shift Narrative Log password security by:

. Not divulging their password to other individuals

. Periodically changing their password in accordance~ with existing Corporate Information Security (CIS) Guidelines.

. Shift Narrative Log entries should only be made by the person logged on to the terminal. Other Operators may make an entry for the Operator logged on the terminal provided that they add their initials at the end of the entry. No entry shall be made without the knowledge and consent of the logged on operator.

. The Shift Narrative Log shall be printed at the end of each shift. The printed Shift Narrative Log is the legal record of plant operation and events. The Shift Narrative Log database contains no legal records.

. The printed Shift Narrative Log shall be reviewed for accuracy and t j completeness and signed by tne Shift Manager.

. If a computer system malfunction prevents log entries using the Shift Narrative Log program, the Shift Narrative Log shall be maintained handwritten using blank forms available in the Shift Narrative Log Notebook in the Shift Manager's office. The handwritten logs may be entered into the Shift Narrative log program when the program becomes available or.the handwritten log may be substituted for the printed Shift Narrative Log.

3.0 Control Operators Log Balance of Plant Log - normally maintained automatically by the Plant Process Computer. If the computer is unavailable, the Control Operator shall maintain the BOP Log by recording values of parameters listed on Substitute BOP Log Sheets. These values are obtained by reading Control Room instrumentation.

I A. The Radwaste Equipment Operator shall maintain a log in the Radwaste Control Room. The following events and activities are representative of the Radwaste Equipment Operator Log entries:

1. Initial entries shall begin by entering the shift hours and date.  !

2. Startup or shutdown of major equipment.
3. Movement of liquids or resins via radwaste systems.
4. Major radwaste equipment maintenance in progress or completed. -I
5. Problems encountered with assigned or completed work.
