Dear Mr. Morris:

This is to report a condition relating to test

- operation of the Reactor Core Isolation Cooling System on Quad-Cities Muclear Power Station Unit 1.during start up testinr. The Recctor Core Isolntion Coolinr Sy'-tem failed to perform its intended function at a reactor pressure of 950 psic. This incident 1-72-5 is reported as an abnormal occurrence.

Incident Descrintion 'and Immediate Action - At 4:30 p.m.

on April ll, 19 7 2, St artup Tes t lio, 14 on the Reactor Core Isolation Coolinr System was being performed at rated reactor pressure. The "RCIC" sys' tem failed to deliver

ij f ~ Inventication and Corrective Acticn - Investigation revealed

I th at the electrical to hydraulic transducer was not f un c tion--

ing' properly. This unit is called the EGR ACTUATOR in the Des cription Manual GEK-9597 - Page 27-16. The response was slow and believed to be due to shorted turns on a magnetic amplifier. A new EGR unit was obtained, tested, and RCIC was declared operable on April 18, 1972.

Dr. Peter A. fiorris April 19,1972 Conclusion - The Reactor Core Isolation Cooling System was tested in accordance with the Quad-Cities !!uclear Power Station Startup Test' Instructions and was found deficient.

The cause of the incident 'f as a defective EGR unit which was replaced, tested, and the system was declared operable.
