Conditions Prior to Occurrence Unit 3 at greater than 90% power.

Description of Occurrence:

During a routine instrument surveillance test, LS-3-23-91A failed to operate .

Designation of Apparent Cause of Occurrence:

The switch assembly flapper arm was found to interfere slightly with the switch cru Analysis of Occurrences . . - ..

L5-3-23-91A and LS-3-23-915 j condensate storage tenk to tha suppression pool. operate independently to sway the HFC and found to. function properly. LS-3-23-918 had just been tested Because the failure of LS-3-23-91A to operate would not have implications interfered with the proper operation of the system, the safety are minimal.
