l During a routine surveillance. test of the IRM system the.

j channel "H" IRM would not respond below 5% on the 125% scale when a 7 l simulated ciectrical signal was applied to its input. This malfunction l would render the IRM downscale rod block from channei "H" inoperable.

Designation of Apparent Cause of Occurrence:

An investigation by the instrument technician revealed that the amplifier and attenuator circuit board was generating an oscillation whose magnitude prevented the channel electronics from responding to a low value signal such as 5% on the 125% scale.

Analysis of Occurrence: .

Due to the redundancy of the IRM channels (any one of the eight channels can produce a rod withdrawal block) and the immediate verification of the operability of the other seven channels, this occurrence has minimal safety implications.

A defective Zener diode was found to be the cause of the mal- f i

function. The diode was replaced, "in kind" and the IRM channel was returned to service.
