., ; SUPPLEMENTARY INFORMATION

-1. Report No.: 50-302/77-80E .

2. Facility: Crystal River Unit #3
3. Report Date: 4 August 1977
4. Occurrence Dates 27 July 1977
5. Identification of Occurrence:

Unplanned release of radioactive gas contrary to Environmental Technical Specification 5.6.2.A.

6. Conditions Prior to Occurrence:

Mode 1 power operation.

7. Description of Occurrence: e At 0950 high radiation alarms exceeding the Administrative limits were received from i radiation monitors RMA-2 & 3. The Auxiliary Building was immediately evacuated and invnstigation revealed the concentrated storage tanks loop seals had blown, causing an unplanned radioactive gas release. The maximum instantaneous release rate at the peak of the discharge profile was .066% of the most restrictive release limit set forth in ETS 2.4.2. A(1) . The loop seals were re-established and when the high radiation alarm cleared, the supply air to the Auxiliary Building re-started and ventilation resumed..
8. Designation of Apparent Cause:

Blown loop seals of the concentrated waste storage tanks causing a radioactive gas release to the Auxiliary Building.

9. Analysis of Occurrence:

4 The health and safety of the plant and public were not affected, as the maximum instantaneous release rates were not exceeding.

10. Corrective Action:

The blown loop seals were re-established and surveillance of them increased.

i

) 11. Failure Data:

I This was the fourth occurrence of an unplanned release or1ginating from the waste gas system as reported by LER's77-49E, 77-61E, and 77-73E I
