S SUPPLEMENTARY INFORMATION Report No.: 50-302/79-013/04L-0 Facility: Crystal River Unit r/3 Report Date: 27 February 1979 Occurrence Date: 23 January 1979 (discovered 22 February 1979)

(determined at plant 26 February 1979)

Identification of Occurrence:

Contrary to Environmental Technical Specification 3.1.5, water samples were not obtained for January from Test Wells 1, 3, and 4, the discharge canal directly to the North of the ponds and the Chemical-Industrial Waste Water Ponds themselves within one month (+25%) of the December 13, 1978 samples.

Conditions Prior to Occurrence:

N/A Description of Occurrence:

The January samples were not collected by January 21, 1979, (December 13 plus 31 days plus 25% of one month).

Designation of Apparent Cause:

A new 1.ab director had been appointed in January and was not properly infor d of the procedure.

Analysis of Occurrence:

The longer than allowed period between samples did not result from oparation of Crystal River Unit #3 nor is operation of the plant affected by it.

Corrective Action:

The new lab director has been informed of the proper time restraint and will be fully responsbile to see that it is followed.
