2. Facility: Crystal River Unit #3 -
3. Raport Date: 25 July 1977

.

5. Identification of Occurrence:

,

Unplanned release of radioactive waste gas costrary to Environmental Technical Specification 5.6.2.A.

6. Conditions Prior to Occurrence: .

'

Mode 1 power operation.

7.

Description of Occurrence: 3

,

At 2017 while venting the makeup tank to the waste gas lov pressure header, an i unplanned radioactive gas release to the Anrflf ary Building occurred. The maxi- .:

mum instantaneous release rate at the peak cf the discharge profile was .0063% "

i of the most restrictive limit set forth in Specification 2.4.2.A(1). When the radiation alarms cleared, the supply air was re-established to the Auxiliary Build-ing and ventilation resumed.

,

S. Designation of Apparent Cause:  ;

i

,

Unknown at this time.

i

9. Analysis of occurrence:

The healch and safety of the plant and general public were not affected as the j maximum instantaneous release rates were not exceeded.

10. Cort active Action:

A concerted effort by plant enr' ag staff involving piping checks is underway to establish the cause, in ord' , define necessary corrective action. As yet, i no conclusion has beau reacher j l

11. Failure Data:

This was the third occurrence of an unplanned release originating from the waste gas system. See LER's 77-49-E dated 14 June 1977 and 77-61E dated 6 July 1977.
