On corresponding with the vendor, National Acme Company, they

          \p L The vendor recommended the following to avoid the possibility of a similar type of failure
: 1. Check snap-lock switches with slow lever arm movement for i                                similar type of failure.
: 2. Tag all National Acme snap-lock switches with a caution note to read:
Caution - when assembling liner onto shaf t, lever must be held stationary so that shaft does not rotate as pipe plug is tightened.

The above mentioned items have been completed on both Units 2 and 3, and are expected to minimize the potential for similar failures in the future.
