Dear Sir:

The above References (a), (b) and (c) requested that licensees submit information regarding materials supplied by Piping Supplies, Incorporated (PSI) at Tolsom, New Jersey and West Jersey Manufacturing Company (WJM) at Williamstcwn, New Jersey and to request that licensees 1) take actions to assure that materials comply with ASME Code and design specification requirements or are suitable for their intended service, o'; 2 ) replace such materials.

Adelitionally, Supplements 1 and 2 to the Bulletin (Ref erences (b) &

l (c)) providad additional information and required that holders of full power operating licenses report the results of their records review, testing, and analysis performed as of the date of the second supplement.

Comonwealth Edison has completed its review pursuant to the request outlined in the Bulletin and Supplements for Dresden, Quad Cities, Zlun, LaSalle County, Byron and Braidwood Stations.

The following Attachments provide a sumary of that review.

l Commonwealth Edison Response to Bulletin 88-05 Nuclear Regulatory Commission (NRC)Bulletin 88-05 required Commonwealth Edison to submit information regtrding materials supplied by Piping Suppites, Incorporated (PSI) and West Jersey Manufacturing Company (HJM).

The Bulletin also requested that licensees take action to assure that msterials comply with ASHE Code and design specifications or are suitable for their intended service. A description of Commonwealth Edison's program for compliance with the requested actions of Bulletin 88-05 as modified by Supplements I and 2 follows.

The scope of review at Commonwealth Edison's Nuclear stations consisted of A$HE Section !!! and/or safety related flanges and fittings, for our three never plants (Byron, Braidwood and LaSalle) the search involved original

construction as well as operational replacements and modifications.

Zion Quad Cities ano Dresden were all operational prior to January 1, 1976. Thertfore the record review encompassed Section !!! and/or safety related Mdifications and replacements only.

For all plants the record search it.volved a two step process.

First, l

purchase and receipt documents were reviewed to determine if any HJM or PSI i

flanges or fittings were received. Next, these identified components were located in the plant.

An itemized list of received components is contained in Attachment A.

l At the time of receipt of Supplement 2 our record review status was as

l follows:

l l

Braidwood - The record search focused on specifications that could have l

supplied flanges or fittings.

These included site purchased pipe fittings and j

Q flanges, flanges for valves, pumps, electrical penetrations, flex hoses NSSS I

equipment and skid mounted equipment. Documents not yet reviewed involve NSSS

[

l and skid mounted equipment which are currently stored with the vendors.

As of August 5th, 527 items were identified as being received at j

Braidwood. One hundred sixty five flanges were found on the shelves. One i

i hundred seventy eight flanges are installed in safety related systems, 50 of

(

l which were declared inaccessible for testir'. The balance of items, 184 l

flanges, are thought to be installed in nonsafety-related applications or have i

been discarded.

Since all safety-related bolting connections were reviewed t

for HJM and PSI materials, the unaccounted for flanges are not installed in safety-related applications.

Overall, Braldeood's review identified only HJM material (no PSI material) i being supplied to Braidwood Station. Of the HJM material, only flanges were l

identified either as individual componentr, or as subparts of flexible hoses, j

Byron - The review of records at Byron is complete with the exception of i

the NSSS and some skid mounted equipment records which are not available for I

revMw.

In addition, portions of the station's Quality Receipt Inspection i

l (QR.i reports have not been reviewed for HJM/ PSI fittings on multi item equipent (e.g. pumps with attached flanges),

17 of these items were in stock, 93 installed in the plant of which 18 are inaccessible for testing.

l The balance of items, 7 flanges, are not installed in any safety-related systems. All items are HJM produced flanges.

LaSallt - The record review of construction and modification documents l

that may contain flanges and fittings is couplete. General Electric has substantially completed their review of NSSS related purchase orders and found i

no components furnished by the suspect manufacturers. A few items require l

additional vendor contact.

These items are not expected to contain HJM or PSI components.

l To date LaSalle has identified 132 items received 42 of these items were in stock. 60 items are installed with 17 1ccated in inaccessible areas of the plant. The remaining 30 items have been vertfled as not installed in l

safety-related systems.

Qf11 din - The document review at Oresden involved purchase orders dating back to January 1, 1976.

Since Dresden is a plant built to the ANSI B3).1 Code many items are received with Certificates of Conformance (C of C's) rather tlian i

Certified Material Test Reports (CMTR's).

This factor complicated the search since j

the original manufacturer was not always listed on the C of C document, j

I Based on the review of P.O.'s with documents traceable to component

[

manufacturer. Dresden has identified 12 HJM flanges. These flanges are installed I

in the plant but because of the temporary suspension of 88-05 activities their l

{

location has not been verified.

l i

Quad Cities - The record review at Quad, similar in scope to that at Dresden, uncovered 77 HJM flanges.

Five of these items are in the storeroom with the other j

l 72 located in the plant. Because of the temporary suspension, the accessibility of j

these flanges for testing has not been determined.

Zinn - Similar to Dresden and Quad, the review of station and contractor f

purchase and receipt documents extended back to January 1, 1976.

The search identified 244 HJM produced items.

Fifty-two were on the shelf, the remaining 192 have yet to be verified as installed.

l To date, the only 88-05 related items identified at Commonwealth Edison f

Nuclear Stations have been HJM flanges, j

In addition to document review, all stations have compiled lists of vendors, I

equipment and specifications for which CMTR's or other forms of traceability are I

[

not available on site.

[

_ TESTING Our testing program began on June 9th, 1988 with Commonwealth Edison's System Haterial Analysis personnel performing EQUOT!P hardness checks on all 165 in-stock i

flanges at Draidwood Station, r

j Initial results indicated a problem with the values obtained with heats LO900, 5448782, CEX, T433, 1855 and 958. Since these were small, light weight flanges and f

difficult to stabilize during testing it was suspected that the EQUOTIP readings l

were inaccurate.

g One flange from each of these heats was brcught into Commonwealth Edison's l

System Material Analysis Department (SMAD) Metallurgy Lab for Rockwell B hardness

}

checks.

Page 4 All derivsd Brinell hardness values from the lab tests on these six flanges l

exceeded the EQUOT!P ot*ained values.

(See Table 1).

Five of the sample flanges were within the hardness value acceptance range and l

as a result the remaining 72 flanges of those 5 heats were deemed acceptable. Heat LO900 although showing an improved hardness value was stiil slightly below the established lower bound.

Subsequent tests (Table 2) performed by Charles Kawin Metallurgical Laboratories confirmed that the strength of LO900 was indeed below SA 105 requirements.

In addition, a macro-etch of the flange revealed that the flange had been machined from plate rather than forged as required by the material specification.

Attachment B contains Edison's hardness test data.

The listing does not enntain hardness values for 76 of Braidwood's HJH flanges.

This is because 72 flanges were qualified by tests per ormed on lab samples and 4 flanges, ahhough tested, are stainless steel for which hardness is not descrictive of mechanical i

properties.

Those items with a "*" preceding thi Station I.D. are items which were tested i

in the lab.

The hardness values were obtained by converting from Rockwell numbers l

ustig the EQUOTIP conversion chart.

Su'nmarizing our testing information, of the 165 stock flanges tested at i

Braidwood. 2 items from heat LO900 were declared nonconforming and flange #BR-006 (Line Item BH-009) from heat 217243 tetted above the high value of 187.

l Byron tested a total of 91 flanges,17 from stock.

Fif ty-seven of these are 1

acceptable and 34 are outside the range of 137 - 187.

Four of these 34 tv.ted high.

LaSalle had 62 of their HJM flanges tested. Nineteen of these items were from stock.

Forty-seven test results were acceptable, 15 tested outside the normal t

range.

MGIE ERING ANALYSIS In the course of our records review and test program several items were found l

to be nonconforming or inaccessible for testing.

Based on the requirements of the original bulletin and Supplement 1, all installed nonconforming components and inaccessible components were reported to the NRC by phone within 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> of their discovery.

Engineering evaluations were performed for all discovered flanges (LaSalle 1&?,

Byron 1&2 and Braidwood 1&2) that were found to be nonconforming (hardness testing) or inaccessible for testing as required by the Bulletin 88-05.

Justifications for Continued Operation (JCOs) were prepared for each component.

Components identified or located after the issuance of Supplement 2 were not I

I tested nor had JCOs prepared.

In addition, a generic engineering evaluation was performed for flanges.

This generic evaluation provided, with a few size and system exceptions, justification for continued operation for LaSalle Units 102, Byron Units 1&2 and Braidwood 1&2.

These generic evaluations were perforr'ed assuming lower bound yield and ultimate values of 27.000 rsi and 44.000 psi respectively.

Thegenericjustificationis also applicable for many of the flunges in Dresden, Quad Cities, and Zion.

The generic analysis performed for these stations was more conservative because l

individual piping design pressures and temperatures were not available on a j

detailed line by line computerized data base.

It is expected that a detailed evaluation of any 7ecific item not covered by the generic justification would also be acceptable, j
