CEOF Off-Hours Activation (August 4,1998)
Comed activated the CEOF as a result of an actual classification of an Unusual Event (UE) by Byron Station on August 4,1998. The activs. ion was initiated conservatively by Comed Electric Operations. Activation of the CEOF is required for an Alert or higher classification, but is not required for an UE. A review of the activation procedure used by the Electric Operations dispatcher indicated that a note in the procedure misled the dispatcher to conservatively activate the CEOF at the UE classification.
Tho UE was classified at 0413 on August 4,1998. The required minimum staff arrived at the CEOF in 68 minutes from the classification time. This exceeds Comed's commitment to staff the CEOF within 60 minutes of classification time.
One minimum staff position failed to respond in the 60 minute time frame. The cause of the delay was a longer than anticipated period between classification of the event and notification of the CEOF rasponders. Seventeen minutes elapsed between the classification of the UE and when the responders were notified.
This time is generally expected to be about 10 minutes. Of the 17 minutes described above, it took 13 minutes from the time that the notification form was approved until the actual notification call from Byron Station to Electric Operations was completed. Four minutes elapsed between the time that Electric Operations received the notification call and when the responders received the notification page. Drive times by responders were within expected results. The 15 minute regulatory requirement to notify the State of Illinois was met.
Comed also committed in the referenced letter to conduct drive-in drills every 6 months until 3 successful drills were completed. Since this drill is considered to be unsuccessful, Comed will conduct at least 3 more drive-in drills every 6 months.
Corrective Actions:

- 1. The Electric Operations procedure was revised to minimize the possibility of unnecessarily activating the CEOF at an Unusual Event classification.
- 2. CEOF activatin will be modified to further reduce the CEOF staffing time by performing reg oder notification in parallel with the State notifications.

September 11,1998 U. S. Nuclear Regulatory Commission Page 3 Responsibility for responder notification will be shifted from Electric Operations to the affected station's control room. This is a change to the process previously described in Attachment G of the referenced letter. We expect this change to improve the notification time of the CEOF responders.
L Augmentation Phone Drills Eleven augmentation phone drills have been conducted between June 1,1998 and August 31,1998. Nine of those drills resulted in estimated staffing times of less than 60 minutes. Two drills conducted in June resulted in failure to attain minimum staffing within the required 60 minutes. In each case, the result was -
due to a single position not being able to respond in 60 minutes. Nuclear Generation Group (NGG) Operations management counseled the involved perconnel. Subsequent performance indicated this corrective action was effective. The last 8 augmentation phone drills resulted in satisfactory or strong i
drill evaluations.
