            Station. The purpose of,the inspection was to review a number of outstanding
            items, particularly the measures being implemented to ensure the radiological
            safety of personnel working in the drywell during fuel movements in the reactor
            vessel and refueling cavity. The inspection was also conducted to review
            radiological controls in connection with the 12R refueling outage, and also to
            review the circumstances surrounding two recent occurrences at Oyster Creek.
            These were the spill of radioactive liquid in the yard of the New Radwaste
            Building and the inhalation of radioiodines by a number of workers following
            removal of the reactor head from the vessel.
            Within the scope of this inspection, two apparent violations were identified.
            The violations involved two instances of failure to follow procedures and a
            failure to perform an adequate survey. In addition, weaknesses were identified
            in your air sampling and analysis program. These weaknesses have been previously
            identified under similar exposure con (itions. Details are provided in the
            enclosed Appendix A and the enclosed report. You are required to respond to the
            violations in accordance with the instructions contained in the enclosed
            Appendix A.
