APPENDIX A NOTICE OF VIOLATION GPU Nuclear Corporation Docket No. 50-219 Oyster Creek Nucle 1r Generating Station License No. DPR-16 As a result of the inspection conducted on October 3-7, October 20 and October 31 - November 4, 1988, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C, Supplement IV) the following violations were identified:

Contrary to the above, on October 22 1988, two workers entered the drywell to pin hangers in preparation for shielding installation. The work was performed in a contaminated area without prior survey of the contamination levels in the area and the associated radiation hazards that may have been present.
This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, GPU Nuclear Corporatton is hereby required to submit to this office within thirty days of the date of the letter which. transmitted this Notice, a written statement or explanation in reply, including: (
the corrective steps which have been taken and the results achieved; (2 ) corrective steps which will be taken to avoid further violations; and (3) the ate when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
