4. Occurrence Date: 15 February 1977

5. Identification of Occurrence:

Fail.ure to monitor correct pilot cells of batteries 3A and 3B.

6. Canditions Prior to Occurrence:

2 Plant was in initial startup configuration from Mode 5 (Cold Shutdown) to ,,

Mode 1 (Power Operation) up to 25% power. At the time of discovery, plant was in Mode 5.  ;

7. Description of Occurrence:

Incorrect pilot cells were used for weekly battery su: ve111ance tests on 3 1/12/77, 2/2/77, and 2/9/77. Quarterly battery survr illance performed on 1/7/77 determined that ce' e A-99 a,' B-106 should ba used as the tested (pilot) cells during week atte f ,urveillance. Onlis A-3 and B-107 were incorrectly used as pilot . is.

8. Designation of Apparent Cause of Occurrence:

As an aid to pilot cell identification, personnel hai pasted stickers on pilot cells the previous quarter. This was not a requirerent and was not covered by procedure. After the current quarterly surveillancs, the stickers were left on the previous quarter's pilot cells. This caused confusion and the incor-rect cells were monitored.

9. Analysis of Occurrence:

There was no impact on plant or public safety, in that correct pilot cells demonstrated operability on two intervening weeks and, again, immediately upon discovery.

10. Corrective Action:

Indicators were promptly placed on proper pilot cells. Revisions have been initiated to the appropriate Surveillance Procedures (SP-520 and SP-521) call-ing for removal and proper placement of the indicators upon determination of new pilot cells by the quarterly surveillance. The Plant Review Committee has favorably reviewed the changes which will be issued prior to' the next quarterly surveillance on 7 April 1977. This should preclude recurrence.
