This refers to the routine safety inspection conducted by Messrs. E.~ Collins.
and D. Lew for the period April-2-29, 1989, at the Oyster Creek Nuclear Gene-
rating Station, The inspections consisted of document reviews,; interviews ~and'
observations of activities as documented in the enclosed inspection report.
The findings were discussed with you and other members of your staff at the
conclusion of the inspection.
This~~ report details two events in which plant activities were not con' ducted in
accordance with approved station procedures. Paragraph 5.0 discusses a surveil-
lance test where procedural' steps were difficult to follow and all procedure
requirements were not implemented. Paragraph 7.0 discusses' an event where sys -
tem operating procedure requirements were not implemented. Review of the un-
derlying causes for these failures indicate that the licensee's policy on pro-
cedural adherence is not being uniformly applied. Increased management atten-
tion is required to ensure effective implementation of this policy.
