I hro cooling towers simultaneously inoperable.

Conditions Prior to Deviation:

Unit 3 operating at full power. Unit 2 off for a refueling outage. The 'A' cooling tower was out of service due to a previous lift ptanp bearing failure. The 'B' cooling tower was fully operable during the outage. River flow vas about 23,000 CFS.

Description of Deviation:

The 'C' cooling tower lift pump tripped, making two cooling towers inoperable.

Designation of Apparent Cause of Deviation:

A flow switch indicated loss of lube water to the lift pump.

The indication was caused by accumulation of silt in the lubricating l i

water lines. {

Analysis of Deviation:

Differential temperatures between the Pennsylvania-Maryland State Line and Holtwood Dam remained within Pennsylvania State limits.  :

Single unit operation and the short duration of the outage minimized l the thermal impact of this deviation.  ;

Corrective Action: f The lubricating water lines were flushed and the 'C' lift pump and cooling tower were returned to service on 4/28/76. p Failure Data: l:

Several previous similar occurrences.
