Identification of Occurrence l
f At approximately 4:50 AM on Octoh** 20, 1356, with Unit 2 in cold shutdown and linit 3 in the refuel mode, tne diesel driven fire pump failed to start i
automatically during the performance of a surveillance test.
l Since the motor driven fire pump was out of service for modifications, the l
failure of the diesel driven fire pump to start rendered both fire pumps inoperable and reportable pursuant to the referenceu Technical Specification.
t Apparent Cause of Occurrence The cause of the pump start failure is under investigation. Yhe results of

- s 10:' Ar. W. 1. Russell Page 2 Actions Taken The diesel driven fire pump was subsequently tested successfully and returned to service at approximately 5:00 AM on 10/20/88. Additionally, the auto-start feature of the pump was tested several times on the afternoon of 10/20/S8 and functioned satisfactorily each time.
