REPORTABLE OCCURRENCE - Diesel Driven Fire Pump Out Of Service At Peach Bottom Atomic Power Station

REFERENCE:

Peach Bottom Units'2 and 3 Technical Specifications 3.14.A.2 This'special report, involving _the inoperability of the diesel driven fire pump, is being submitted pursuant to the referenced Technical Specification.

Identification of Occurrence On July 10, 1989, with Unit 2 operating at 68% power and Unit 3 in the refuel mode with the core offloaded, the diesel driven fire pump was out of service in excess of seven days. This condition is reportable pursuant to the referenced Technical Specification.

Cause of Occurrence

.The pump was rendered inoperable on July 3, 1989 during performance of the monthly functional test. A coolant hose became disconnected from the engine block during the test, draining coolant from the engine and heat exchanger, resulting in engine damage. It is believed that the hose clamp was snug but not tight and became disconnected the next time the engine was operated.

Actions Taken The motor driven fire pump was verified operable and was tested every 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />

.in accordance with Technical Specification 4.14.A.2. The diesel vendor performed inspection and repair of the engine.

practical'. -The monthly surveillance test has been revised to require verification of no coolant. leakage immediately.after starting.the diesel.
