UNIQUE REPORTING REQUIRDENTS SEISMIC MONITORING INSTRUMENTATION INOPERAB'J FOR MORE THAN 30 DAYS

Technical Specification 3.15 B and 6.9.3
Dear Mr. Haynes:
In accordance with the. subject Technical Specification reporting requirements, this is to advise you that the triaxial accelerometer device installed in the Unit 2 RCIC pump room has been inoperable since June 4, 1982.
Apparent Cause:
' ie triaxial accelerometer in the RCIC room was damaged by Tl a buildup of water in the accelerometer housing shorting.the detectors and power supply. Attempts made to repair the traixial accelerometer were unsuccessful.
The source of water is believed to have been a flood in the BCIC room that occurred prior to the date 'of inoperability.
Corrective Action:
,3 Three of four triaxial accelerometers remain operable..These are located on the Unit 2 containment foundation,. Unit,2 r,efueling floor, and the E-3 emergency diesel generator roon' -;

A replacer.ent detector for the RCIC room is expected from the manufacturer within a month.
Installation and return to service is' expected by August 31, 1982.
