Analysis of, Occurrence:- During the re-evaluation. of concrete block walls in response to IE Bulletin 60 11, four walls, identified as walls 6d.1 to 68.4,

!         were found to be unstable under the effects of tornado l          d gressurisation and cardox injection. These are the computer                                                                                                                      l room walls in the cable spreading room. Collapse of the walls could affect safety-related cables which pass through or are in cloce proximity to the walls.

05Frective Actiont- '. Until a permanent ~ modification. i's completed, the computer room doors will be- blocked open..approximately 3 inches. ':'his opening will prevent wall failure ,due ' to either tornado depressur12ation-or cardox injeotion. The permanent' design modification requires insta11'ation. of vente cbove the computer room doors with ducts added to raise the vent - opening to the t'op of 1Fhe computer room.
