Peach Bottorn Atomic Power Station - Unit 2 & 3 Special Report for a Non Valid Failure of the E-4 Emergency Diesel Generator on 2/11/94

REFERENCE:

Peach Bottom Atomic Power Station Technical Specification (Tech Spec) 4.9.A.1.2.L and M This Special Report is submitted pursuant to the requirements of Tech Spec 4.9.A.1.2.M. This Surveillance Requirement requires reporting of all Emergency Diesel Generator (EDG) failures, valid or non valid, within 30 days. This report is i required to include the information recommended in Regulatory Position C.3.d of Regulatory Guide (RG) 1.108 " Periodic Testing of Diesel Generator Units as Onsite Electric Power System at Nuclear Power Plants", Revision 1, August 1977.

On 2/11/94 at approximately 0300 hours0.00347 days <br />0.0833 hours <br />4.960317e-4 weeks <br />1.1415e-4 months <br />, with Unit 2 and Unit 3 at approximately 100% power, the E-4 EDG became inoperable during the performance of a monthly Surveillance Test (ST)-O-52-204 2 "E4 DIESEL GENERATOR SLOW START AND FULL LOAD TEST" when a Nuclear Plant Operator inadvertently bumped the Voltage Shutdown Button while attempting to acknowledge an annunciator. This action shorted the generator field and caused voltage and load instabilities. The E-4 EDG output breaker was manually tripped open from the Main Control Room and the diesel was secured.

I A Maintenance inspection was initiated to deterrnine if generator damage may have occurred due to this event. Extensive testing identified that no engine or generator damage had occurred. The event has been discussed with the involved individual _

U. S. Nuclear Regulatory Commission Page 2 Operations personnel. On 2/13/94 at approximately 1040 hours0.012 days <br />0.289 hours <br />0.00172 weeks <br />3.9572e-4 months <br />, the E-4 EDG was returned to service and declared operable.

The E-4 EDG failure was classified as a non valid failure using the guidance of RG 1.108, Revision 1,1977. Because this occurrence was classified as a non valid failure, the current surveillance testing interval will remain at once per 31 days which is in conformance with RG 1.108, Revision 1, Section C.2.d.

The E-4 EDG was considered inoperable for 2 days 7 hour8.101852e-5 days <br />0.00194 hours <br />1.157407e-5 weeks <br />2.6635e-6 months <br /> and 40 minutes. The remaining EDGs were operable and would have provided adequate AC power to safety related loads in the event of an actual loss of offsite power.
