REPORTABLE OCCURRENCE - Both Fire Pumps Out Of Service At Peach Bottom Atomic Power Station

Peach Bottom Units 2 and 3 Technical Specifications (TS) 3.14.A

Dear Mr. Russell:

This special report, involving the simultaneous inoperability of both fire pumps, is being submitted pursuant to the referenced Technical Specification and satisfies the 14 day reporting requirements in accordance with TS 3.14.A.3b and the 31 day reporting requirements in accordance with TS 3.14.A.2.

identification of Occurrence:

At 1400 on August 22, 1988, while the motor driven fire pump was i operating, discharge pressure was lost. The pump was immediately secured and declared inoperable. Since the diesel driven fire pump was out of service for preventative maintenance, this occurrence rendered both fire pumps inoperable.

At the time of this occurrence, Unit 2 was in cold shutdown and Unit 3 was in i the refuel mode with the vessel defueled. A 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> notification was made to the NRC Operations Center at 1915 and a written notification was submitted on 8/23/88.

Cause of Occurrence:

The pump failure was caused by a crack that formed in the column pipe flange weld area where it bolts to the pump. This degradation resulted in misalignment of the pump and the subsequent failure of the shaft coupling.

The diesal driven fire pump was returned to service on 8/22/88 at 1855.

The motor driven pump was removed from the pump structure, disassembled and inspected. Since the parts necessary for repair are not available, and the vendor no longer manufactures this particular model, a replacement pump has been ordered.

Plans and Schedule:

The vendor is scheduled to ship the replacement pump on 9/2/88. It is expected that the new pump will be in service by 9/9/88.
