The following reports the observation that lead to the discovery and removal of a heliarc welding outfit in a jet pumo riser for the Unit 2 reactor. This incident is reported as No. 2-72-4.

Des cription of Incident l

Jet pumps flow comparison checks were being performed on the " nit 2 reactor on April 23, 1972 as a final check prior to installation of the steam separator, steam

! dryer, and reactor ves'sel head. This test requires that both reactor recirculation pumos be running at the same speed with the recirculation header crosstie valves closed.

The indicated flow rates on each of the twenty (20) jet pumps are then checked and rhould be at approximately j the same value. The indicated flow rates on jet pumos (10) and (20) were about 50 per cent of the other 18 jet pumps. At 5:30 p.m. on April 28, 1972, the decision f bl was nade to remove jet pump (20) to determine the cause of the flow reduction. Jet pump No. 20 was removed at I p'O .

Review of Peactor Cleanliness The quality control records for the installation, inspection, T and flushing of: Unit 2 jet pumps Numbers (19) and (20); #

recirculation header and reactor vessel were reviewed.

The jet pump riser had been properly installed, inspected, -1 and flushed. Flushing of all the emergency core cooling systems prior to primary systern hydrostatic test in May, Q

4 1971, and the residual heat removal system pump flow rate .:f tests performed in June,1971 provided good flushing of the recirculation piping system. A final inspection of i -

the reactor bottom head after the hydrostatic test and just prior to the installation of the core internals was "g)"

made on July 14, 1971.

Board studied the quality control records, the flow test -

results, and the inspection that followed the jet pump d installation, and as a result of the investiFation the 4 reactor vessel and recirculation system was declared clean 3.c and clear of foreign material.

Approval was given to continue with the startuo test pro- -r grarn on v av 2, 1472 by the Station Feview Board.
