APPENDIX A NOTICE OF VIOLATION Mississippi Power & Light Docket No. 50-416 Grand Gulf 1 License No. CPPR-118 As a result of the inspection conducted on October 19-30, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified.

A.

10 CFR 50, Appendix B Criterica V and the accepted QA Program (MPL-Topical-1A, Revision 2) Policy 5 require that activities affecting quality shall be prescribed by documented procedures and shall be accomplished in accordance with these procedures. Administrative Procedure (AP) 01-S-06-3, Control of System Temporary Alterations, Revision 4, TCN No. 4, paragraph 6.1.2 requires tagging temporary alterations.

Contrary to the above, as of October 21 and 27, 1981, the following -

temporary alterations (electrical jumpers) were not tagged as required by AP-01-S-06-3.

1.

Panel H13-P694, RPS/NSSS Logic Division 4, Terminal AA, contacts 72 to 73 jumpered.

2.

Panel H13-P871, E3F Logic Division 1, Relay Rack T48R10, contacts 6 to 7 jumpered.

3.

Panel H13-P612, Feedwater and Recirculation Instrument Panel, eight potentiometers jumpered across various terminal points.

This is a Severity Level V Violation (Supplement I).

B.

10 CFR 50, Appendix B Criterion V and the accepted QA Program, Policy 5.0, require that activities affecting quality shall be prescribed by documented procedures and accomplished in accordance with thcse procedures.

The accepted QA Program, Policy 6.5.8 and AP 01-S-02-2, Revision 3 require that Temporary Change Notices (TCNs) for permanent changes to procedures shall be incorporated into revisions within six months of issue.

Contrary to the above, as of October 23, 1981, eight examples were i

identified involving TCNs to administrative procedures ranging in date from 7/16/80 through 4/22/81 that had not been incorporated into procedure revisions within six months of issue.

This is a Severity Level VI Violation (Supplement I).

Docket No. 50-416 Grand Gulf 1 License No. CPPR-118 Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) adnission or denial of the alleged viola-tions; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.

The responses directed by this Notice are not subject to the clearance procedures of the Of fice of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
