During the Nuclear Regulatory Commission (NRC) inspection conducted on June 20 l

through July 17, 1993, a violation of NRC requirements was identified.

accordance with the " General Statement of Policy and Procedure for NRC l

Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed below:

Technical Specification 6.8.1.c requires that written procedures be established, implemented, and maintained covering surveillance and test activities of safety related equipment.

Step 96 of Control Rod Movement Sequence 1-6-10400-B1-01 required the reactor operator to withdraw con +rol l

rod 28-53 per procedure 06-RE-SC11-V-0402, Revision 33, Control Rod Seram Testing - Individual Scram - Transient Recorder Auto Analysis Method, from notch 08 to notch 48 prior to scramming the rod.

Contrary to the above, on July 11, 1993, a reactor operator directed the scramming of rod 28-53 from its original position of notch 08 prior to its full withdrawal.

This is a Severity Level IV violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, Entergy Operations, Inc. is hereby required to submit a written statement or explanation to the U.S. Nuclear l

Regulatory Commission, Attn: Document Control Desk, Washington, DC 20555, with l

a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, Grand Gulf Nuclear Station within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a " Reply to a Notice of Violation" and should include (for each violation): (1) the reason for the violation, or if contested, the basis for disputing the violation; (2) the corrective steps that have been taken and the results achieved; (3) the corrective steps that will be taken to avoid further violations; and (4) the date when full compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, an order or demand for information may be isso?d as to why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
