1 ENCLOSURE 1 NOTICE OF VIOLATION Entergy Operations, Inc.

Docket No.: 50-416 Grand Gulf License No.: NPF-29 l

During the Nuclear Regulatory Commission (NRC) inspection conducted on March 14 through April 17, 1993, violations of NRC requirements were identified.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1991), the violations are listed below:

A.

Technical Specification 6.8.1.c requires that written procedures be I

established, implemented and maintained covering surveillance and test activities of safety related equipment.

Performance and System Engineering Instruction 17-S-02-400, Revision 3, Attachment III (Individual Rod Movement Tracking Sheet) required the reactor operator to leave control rod 16-37 L

fully inserted at position 00 after scram time testing performed on March 20, 1993.

Contrary to the above, on March 20, 1993, a reactor operator i

mistakenly withdrew the control rod from position 00 to position 32 after scram time testing of the rod.

Surveillance Procedure 06-0P-lCll-W-0001, Control Rod Operability Testing, Revision 29, Step 5.3.4.a. required that each control rod undergoing its weekly surveillance be moved in one notch, and then be returned to its original position, j

i Contrary to the above, on April 2, 1993, a reactor operator mistakenly inserted control rod 56-45 an additional notch during operability testing of the rod prior to returning the rod to its original position.

This is a Severity Level IV violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, Entergy Operations, Inc. is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, Attn: Document Control Desk, Washington, DC 20555, l

with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, Grand Gulf Nuclear Station within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a

" Reply to a Notice of Violation" and should include (for each violation):

(1) the reason for the violation, or if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
