" Technical Specification 6.7.la requires that written procedures be implemented covering activities delineated in Appendix "A" of Regulatory Guide 1.33, Revision 2, February, 1978. Operations Procedure 10001-C, Section 3.3, specifies Table 1 for inspection criteria when performing rounds. Paragraph 5.0 of 10001-C further states that the operator on duty is responsible for all charts in his area, which includes assuring that each recorder chart is marked daily with the time and date and verification that the chart is operating properly or to initiate an MWO for repair.

Contrary to the above, on December 29, the NRC identified that improper control room rounds were being conducted and inadequate corrective action was taking place with respect to the control room chart recorders. The findings include the following:

1. Failure to verify proper operation and/or mark the fol-lowing recorders daily with the time and date:
a. S/G. steam pressure (1PR-514)
b. Low pressure turbine steam pressure (lPR-6237)
c. Core monitor panel (ll328-QS-CMP)
d. Power range (lNR-47)
e. Rod position insertion limit (lZR-412)
2. Failure to mark the charts listed in Paragraph 1 above following chart adjustment.

This is a Severity Level IV violation (Supplement I)."

RESPONSE TO VIOLATION 50-424/88-61-01 ADMISSION OR DENIAL OF ALLEGED VIOLATION The violation occurred as stated.

REASON FOR THE VIOLATION The violation occurred because of the failure of control room operators to adhere to the requirements of Procedure 10001-C, "Logkeeping," and the inadequate emphasis placed on the requirements by the appropriate operations supervisor.

NRC NOTICE OF VIOLATION 88-61-01 AND GPC RESPONSE CORRECTIVE STEPS WHICH HAVE BEEN TAKEN AND THE RESULTS ACHIEVED:

Shift operators and shift supervision have been counseled on the failure to follow the procedural requirements of 10001-C. An Operations Standing Order, C-89-01, " Operation of Chart Recorders," was issued on January 31, 1989. The standing order re-emphasizes the requirements of Procedure 10001-C, references this violation, and provides additional requirements upon shift operators and shift supervision. The additional requirements are, (1) the control room operators shall check recorders for proper operation once each shift and mark the charts with date and time; and (2) the Shift Supervisor (SS) or the On-Shift Operations Supervisor (OSOS) shall review control room chart recorders each shift to confirm that the operators have properly marked the charts. The aforementioned additional actions will remain in effect until Operations Management feels confident that proper emphasis is being placed on chart recorders and their operability.

CORRECTIVE STEPS WHICH WILL BE TAKEN TO PREVENT RECURRENCE OF THIS VIOLATION:

It is believed that the aforementioned corrective action taken should prevent recurrence of this violation. However, as previously noted, the standing order will remain in effect until Operations Management is confident that the requirements of Procedure 10001-C are understood and are being met. Procedure 10001-C requires the control room operators to check chart recorders for proper operation once each day and mark the charts with date and time.

DATE WHEN FULL COMPLIANCE WILL BE ACHIEVED:

Full compliance was achieved on January 31, 1989, upon issuance of Standing Order C-89-01.
