O'Reilly LER No.82-051 Page Two December 30, 1982 DETAILED DESCRIPTION OF EVENT On December 2, 1982, with the Plant in Mode 1, Surveillance Test Procedure (STP) 145.002 was performed unsatisfactorily on the Waste Evaporator.

The liquid radwaste treatment system is required to be OPERABLE per Technical Specification 3.11.1.3,

" Radioactive Effluents, Liquid Waste Treatment".

PROBABLE CONSEQUENCES There were no adverse consequences resulting from this event as the system was in compliance with the Technical Specification Action Statement.

CAUSE(S) OF THE OCCURRENCE The distillate pump of the waste evaporator package is equipped with an external thermal cut-out switch that is wired into the pump motor circuitry as a start permissive.

The leads for the thermal cut-out switch were found pulled out of the pump junction box, which inhibited the pump start.

The cause for the damaged assembly is unknown.

IMMEDIATE CORRECTIVE ACTIONS TAKEN The Licensee took the following corrective actions:

1) the thermal cut-out assembly was replaced;
2) the circuitry was megged to ensure circuit continuity; and 3) the applicable Surveillance Test Procedure was performed satisfactorily on December 13, 1982.

ACTION TAKEN TO PREVENT RECURRENCE The Licensee plans no further action for this event other than the normal surveillance testing.
