l Mr. James P. O'Reilly LER No.82-006 Page Three October 21, 1982 DETAILED DESCRIPTION OF EVENT Several High Range Radiation Monitors have experienced problems with moisture accumulation in the connector between the detector and the preamp causing the monitor to fail high.

The monitors affected are as follows:

RM-A13 Main Plant Vent (High Range Monitor) which was declared inoperable on September 22, 1982.

RM-A14 Reactor Building Purge Exhaust (High Range Monitor) which was declared inoperable on September 22, 1982.

RM-Gl9A Main Steam Line Loop A (High Range Monitor) which was declared inoperable on September 22, 1982.

RM-Gl9B Main Steam Line Loop B (High Range Monitor) which was declared inoperable on October 11, 1982.

PROBABLE CONSEQUENCES There were no adverse resulting consequences.

The subject monitors are High Range Accident Monitors, and the Plant had not reached initial criticality, thus eliminating the need for such indication.

CAUSE(S) 0F THE OCCURRENCE The cause has been determined to be that moisture is penetrating the seal of the connector.

IMMEDIATE CORRECTIVE ACTION

The connectors on all of the subject monitors were cleaned and dried.

The detectors were replaced on all of these monitors except RM-G19B.

The monitors were then tested and declared operable per applicable Surveillance Test Procedure within their 72 hour8.333333e-4 days
0.02 hours
1.190476e-4 weeks
2.7396e-5 months
alotment.

ACTION TAKEN TO PREVENT RECURRENCE The licensee is currently consulting the equipment supplier to determine if further corrective action is necessary.
