O'Reilly LER No.82-014 Page Two November 1, 1982 DETAILED DESCRIPTION OF EVENT Technical Specification 4.6.1 3.(c) requires the Reactor Building personnel airlock to be verified operational once every six months to ensure that only one door of the airlock can be opened at a time. Surveillance Test Procedure (STP) 115.002, " Reactor Building Airlock Test," addresses this specification. In performing the STP with the Plant in Mode 3, it was discovered that the air pressure equalizing valves would not cycle in their proper sequence. The test was immediately terminated, and the airlock was declared inoperable on October 2, 1982. A maintenance work request (MWR) was initiated for troubleshooting and repair.
The airlock was repaired and returned to operable status on October 3,1982, within the 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> requirement of the Technical Specification Action Statement (a).
PROBABLE CONSEQUENCES There was no adverse consequence as the Plant had not achieved initial criticality and no radiation inventory existed.
CAUSE(S) 0F THE OCCURRENCE A chain drive sprocket that actuates the pressure equalizing valves sheared a key allowing the sprocket to slip on its associated drive shaft. This caused mechanical linkages to move out of sequence.
IMMEDIATE CORRECTIVE ACTIONS TAKEN The key was replaced, and the airlock was tested satisfactory per STP-115.002 within the 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> requirement of the applicable Technical Specification Action Statement.
ACTION TAKEN TO PREVENT RECURRENCE The licensee considers this failure to be an isolated case; and therefore, plans no further action for this event.
