November 19, 1982 s DETAILED DESCRIPTION OF EVENT On October 22, 1982, the Plant entered Mode 2 for initial criticality with the Loose-Part Detection System inoperable with respect to Technical Specification 3.3.3.10. (Specification 3 0.4 is not applicable.)

PROBABLE CONSEQUENCES There were no adverse consequences resulting from this event.

Even though modifications were being performed, the system was in operation and functio,nal.

CAUSE(S) OF THE UCCURRENCE l

Modifications were being performed on the Loose-Part Monitoring System that were not complete at the time the Plant entered Mode

2. Therefore, the applicable surveillances had not been performed.

IMMEDIATE CORRECTIVE ACTIONS TAKEN The modifications were completed on the system, and the applicable Technical Specification surveillance tests were performed on November 9, 1982. Af ter a time period of system operating observation, the system was returned to operable status on November 19, 1982.
