Technical Speci fication 3.3.7.11 requires the Wide Range Stack Gas Monitor to be operable. Upon completion of LOS-PR-M3, Main Stack and SBGT Wide Range Radioactive Gas Monitor Source Check on May 17, 1982, the low range monitor failed to return to normal. This caused the Wide Range Stack Gas Monitor to be inoperable. Rad Chem was informed to draw Grab Samples every 8 hrs and analyze for Noble Gas within 24. f V. PROBABLE CONSEQUENCES OF THE OCCURRENCE At the time of this event, LaSalle Unit One was in cold shutdown, with no irradiated fuel on site. Safe operation of the facility was maintained at all times. VI. CAUSE The cause of this occurrence was a failed photomultiplier tube. This tube was manuf actured by EMI, Model 9856KA. Vll. CORRECTIVE ACTION A work request was initiated to investigate the failure ~of the Wide Range Stack Gas Monitor. Under this work request the photomultiplier tube was replaced and the nonitor will be calibrated during the week of June 7,1982. l [ l Prepared by: Diane Kay 1
