REACTOR OPERATOR AND SENIOR REACTOR OPERATOR REQUALIFICATION EXAMINATIONS Gentlemen:

This is to acknowledge receipt of your letter of December 23, 1985, indicating OPPD's course of corrective action based on the results of the NRC administered requalification examination on November 12, 1985.

'NRC appreciates OPPD's timely response and cooperation in dealing with this matter. NRC concurs with the corrective actions proposed by 0 PPD to retrain operators who failed the examination and to upgrade the requalification program at FCS. To reemphasize the status of license renewals for FCS operators: ,

l. . All renewal applications must continue to be submitted in a timely manner in accordance with 10 CFR Part 55.
2. Until such time that the FCS requalification program is evaluated as satisfactory, all operators applying for license renewal after November 12, 1985, must pass an NRC administered requalification examination.
3. Operators who apply for license renewal in accordance with 10 CFR Part 55 and who have passed a FCS administered requalification examination through December 12, 1985, may continue to exercise their privileges under the current license until an NRC examination can be administered.
4. Operators who have passed an NRC administered requalification examination within one (1) year prior to the expiration of their current license and who have passed all interim requalification examinations will be considered for inrnediate license renewal.

Dear Mr. Martin:

Fort Calhoun Station Operator License Requalification Program On Wednesday, December 11, 1985, the Omaha Public Power District (OPPD) was notified by members of your staf f of the results of NRC license regualification examinations conducted on November 12, 1985. Of eight OPPD personnel examined by the NRC, six failed the NRC written examination. Promptly following this notification, OPPD took the following actions:

(1) Removed those who failed the NRC examination from their assigned license duties, in accordance with OPPD's operator license requalification program.

l (2) Completed grading all OPPD requalification examinations ad-ministered by OPPD during the current refueling outage. A total of fifteen OPPD requa.lification examinations were so administered.

l (3) Dispatched an OPPD management team to your offices. This management team met with you and members of your staff on l

December 12 and 13, 1985.

As a result of item (2) above, one of the fifteen individuals administered the OPPD requalification examinations failed the examination. This individual has also been removed from licensed

shift duties, in accordance with OPPD's operator license requali-l fication program.

December 23, 1985 Page Two As a result of our meetings on December 12 and 13, 1985, OPPD presented a corrective action plan to members of your staff on December 13, 1985. This corrective action plan consisted of the following elements:

Actions Planned for Remedial Group * (to be completed prior to March, 1986)

(1) A detailed review of examination failed with each indivi-dual.

(2) Accelerated training to be provided in identified weak areas.

(3) Frequent quizzes will be provided during training to ensure mastery of learning objectives.

(4) Training on written examination skills will be provided to each member of remedial group.

(5) An audit examination will be provided to each member of remedial group by a qualified consultant to check overall readiness for recertification.

Actions Planned for OPPD's Requalification Program (1) Improve attendance at scheduled requalification training sessions.

(2) Provide performance-based training materials to licensed personnel and to the NRC.

(3) Address the need for improved on-tne-job training (OJT),

especially for senior operator licensed personnel.

  • Remedial Group - Consists of those individuals who failed the NRC requalification examination (six) and the individual who failed OPPD's requalification examination. This is also the group who will be provided accelerated requalification training in identi-fled weak areas, in accordance with OPPD's requalification train-ing program.

As I understand it, yciur staf f has concluded that the OPPD re-qualification examinations administered this fall were of adequate quality, satisfactorily graded, and properly prepared and admini-stered. Although this is a very significant, and not unexpected, positive result for OPPD, I want to assure you that I expect inaprovements in our requalification training process as detailed previously.
