Harrell and T. Reis of A F this office during the period April 16 through May 12, 1990,.of activities authorized by NRC Operating License DPR-40 for the Fort Calhoun Station, and to the d.iscussion of our findings with you and other members of your staff at the -conclusion of the inspection. - Areas examined during the inspection included operational safety verification; . monthly maintenance, surveillance, security, and radiological protection observations; onsite followup of eventi.; and in-office review of reports. Within these areas, the inspection consisted of selective examination of , procedures and representative records, interviews with personnel, and
observations-by the inspectors. The inspection findings are documented in the en' closed inspection report. . . t During .this inspection, it was found that certain of your activities were in- -violation of NRC requirements. :However, in accordance with Section V.A of the - ~ -NRC's Enforcement Policy, no citations are being issued for the violations discussed in paragraphs 5.a, 5.b, and 5.c of the enclosed inspection-report. .For:the violation discussed in paragraph 5.c, your staff failed to revise a
- safety analysis for a field change to a design modification. Although the act
, constituted procedural noncompliance,- the safety significance was minor. It- 1 was' evident-the staff had examined the full effect of the field change.- Fori o the violation identified in paragraphs 5.a and.5.b, your. staff failed to document and report deficiencies as required by'the quality assurance plan. It ', was evident that the deficiencies were elevated to the appropriate levels of management; however, documentation and formal corrective action were lacking. Plant management addressed these issues identified by the inspector in a prompt ,and thorough manner. L It is our understanding that you will incorporate training on the requirements to document and report deficiencies into'the general employee training- curriculum. This commitment is discussed in paragraph 5.b of the enclosed > report. ' We have also examined actions you have taken with regard to previously . , , T identified inspection findings. The status of these items is identified in
